Registration Packets
Registration Packets are available electronically via email or paper copies can be picked up in person at the office. The Packets include the current Student Handbook, Student Information Form, Code of Conduct, Injury/Illness Form, and Media Release.
All forms must be completed, signed, and turned in BEFORE any student is permitted to participate in classes.
Student Information
Each year , every student must have a Student Information Form completed and signed by a parent or guardian, if under 18 each year of enrollment. The Information Form will need to be reviewed and initialed each session.
Class Payments
All students must register and pay for classes to secure their place in the class. No student will be permitted to participate in class if they are not registered and paid in full. Class fees are on the Classes and Prices page.
There is limited space in each class.
Classes may be cancelled due to low enrollment.
Fees and Late Payments
There is a one-time $25 Administration Fee per student. This does not apply to voice or music lessons, art classes, or camps.
Returned checks/non-sufficient funds will be assessed a $35 fee in addition to the payment due. If you have two returned checks, you will be required to pay in another manner in the future.
Convenience Fees will be charged if paying by credit card.
Refund Policy (Classes and Camps)
Classes: If cancellation is made at least one week before the first day of the class session, you will be entitled to a full refund. No refunds will be made less than a week before classes begin, and no proration of class fees will be issued after the class session has started. Class fees cannot be transferred to another class or term.
Camps: Cancellations made at least two weeks before the camp starts will receive a full refund. No refunds will be issued less than two weeks from the date that the camp starts and no proration of camp fees will be issued once the camp has started. Camp fees cannot be transferred to another camp.
Weather/Emergency Policy
If classes are cancelled due to bad weather or an emergency, we will do our best to contact all of our students. We will change Mustard Seed Art Academy’s voicemail message (304) 398-5707 to reflect the cancellation. We will also make announcement posts on all our social media accounts.
However, we encourage you to ALWAYS use your best judgement when driving in poor weather conditions. Please put your/your child’s safety first at all times.
In the event we close due to the weather, we will have a make up date scheduled for all cancelled classes.
Students, parents, and guardians are expected to sign and observe the Code of Conduct which is provided in the required forms. Failure to abide by the Code of Conduct may warrant dismissal from class and/or future classes.
To ensure the health and safety of all our students, please do not come to class sick (fever, vomiting, etc.) or injured.
Students becoming ill or injured during class or while on the premises for Mustard Seed Arts Academy functions will have a parent/guardian contacted immediately by staff.
An Injury/Illness Policy and a Student’s Information Form with the Health and Medical Information section completed must be on file.
No photography or videos may be taken during class sessions. This is to ensure that our students are focused on their lessons and to maintain the safety of our students and instructors.
We will have our studio photographer taking pictures and videos for promotional and publicity use throughout the year.
A Media Release must be on file for every student.
Attendance
Regular attendance is in direct correlation to the student’s progress and understanding of the material. Excessive absences may result in exclusion from performances and events.
Please be prompt and prepared for scheduled class time.
Dance and movement classes will include stretching and a warmup as class starts and it is important for all students to participate for their safety and injury prevention.
Class Cancellation
If classes are cancelled, we will do our best to contact students as soon as possible. Instructors will attempt to contact their students by email and/or phone. Messages will also be sent out via social media.
